  2. Updated CV - internship company included
  3. Updated Cover Letter- internship company included
  4. Offer Letter
  5. Week 1 email to Mentor
  6. APU Internship Supervisory Log sheet

  •Part 2:

  1. Report
  2. Logbooks Week 9-16*
  3. Industrial Placement Report Clearance Form*
  4. Company Supervisor Assessment Form*
  5. Attendance Logs*
  6. Visit Report Form

  *all documents relevant to company must be signed & stamped by the company
